Five Wishes: The Gift of Conversation

Many individuals recognize the importance of discussing their end-of-life care wishes, yet few have had these conversations. Have you had that talk? In this session, you will discover more about advanced care planning and how to share this vital conversation with those who matter most in your life. The emphasis will be on:
- How to initiate advanced care planning discussions with loved ones.
- Choosing the right person to speak for you when you cannot communicate for yourself due to a health crisis.
- The importance of updating your documents as your life situation changes.
All attendees will receive a complimentary Five Wishes document, which will guide you through the necessary steps and considerations. Once completed and signed, this document serves as a legally valid advance directive. Join us to ensure that your end-of-life wishes are honored.
This program will be led by Kathy Baillargeon, the Executive Director for Hospice Volunteers of Hancock County, and a Five Wishes Certified Presenter.
